What is the key difference?

C-5M Super Galaxy: The high-mounted wing clears a full-width cargo hold. Nose and rear ramps can open at the same time, enabling vehicles and freight to load from either end.

An-124 Ruslan: A double-deck fuselage separates the crew area from the main cargo hold. Nose and rear doors allow vehicles to pass through, while the landing gear can lower the forward fuselage.
